Summary of Position: The primary responsibility of the Case Manager is to initiate, organize, plan, develop and ensure implementation of direct services to assigned clients and their families. All service plans will be based on a strength-based approach using the wraparound model.
- Serve as a member of the youth’s Juvenile Justice Case Action Team, attend case conferences and work with the team cooperatively in the best interest of the youth and family. The RAC will develop an initial Individualized Service Plan (ISP) based upon the results of the initial youth/family, community assessment. Ensure that all assigned clients and their families receive services, advocacy hours, contacts, and specified activity monies as required by the Program Director and the clients service plan.
- Develop a supportive relationship with each client’s family. Involve the family in the development of meaningful activities. Ensure that the parents know the whereabouts of their children while they are participating in program activities.
- Develop with each client a set of objectives to be achieved during the client’s participation in the program and following discharge.
- Ensure that each assigned client is participating in an appropriate education/vocational program, a supported work program, or unsubsidized employment. Ensure that the client attends school and/or work on a regular basis.
- Under the Program Directors supervision, involve the appropriate community professional resources as determined by the client’s individual needs and the service plan.
- Assist each client and family in identifying resources or persons who can provide continued support beyond termination of advocacy services and develop linkages to those community resources and persons.
- Respond to emergency situations or special incidents by involving the client or the client’s family as the need arises. Attend court hearings as required.
Qualifications/Requirements: Master’s Degree generally preferred; At least one year experience working with children, adolescents and families; Knowledge of community resources
Valid driver’s license. Minimum Auto Insurance Coverage of $100,000 per person and $300,000 per accident bodily injury liability and access to a reliable transportation/vehicle.
